Â鶹TV offers one of the largest behavioral health outpatient programs in Santa Clara County. Our outpatient programs serve adults who are living in the community or in our independent and supportive housing programs. Your appointments will take place at one of our four locations in San José or Mountain View.

You receive an individualized care plan after a thorough assessment by one of our clinical staff and a review of your history. Your care team may include a housing specialist inside and outside of Â鶹TV. Your dedicated case manager supports you throughout our time in the Adult Outpatient program.

We take a whole person care approach to support your mental, emotional, physician, and social well-being. You may qualify for some or all these services:

  • Diagnosis
  • Employment services
  • Independent or supportive housing
  • Individual or group therapy
  • Psychiatric care
  • Skill development
  • Symptom and medication management

What to Expect:

    You are referred to our program by a case manager or another care provider.

    Santa Clara County processes your application to our program.

  • 3.

    We review your case and talk with you to learn about your behavioral health history.

  • 4.

    You are assigned a care team who helps you access all the services you qualify for.

All referrals for the Adult Outpatient Program must come through Santa Clara County.

Who Is Eligible?

To be eligible for our Adult Outpatient Program, you must meet the following criteria:

  • You are at least 18 years old.
  • You have a behavioral health diagnosis.
  • You have active Medi-Cal.
  • You live in Santa Clara County.

Certification Information

Providers with a variety of certifications work in our programs, including case managers, nurses, therapists, and psychiatrists.

Outcome

We currently serve over 1,500 clients across four Santa Clara-based sites. Adult outpatient care helps clients manage behavioral health conditions and crises, so they can stay out of the justice system and avoid hospitalization whenever possible.

Day in the Life

As a client enrolled in our Adult Outpatient Program, you have access to a case manager who helps you navigate the services you qualify for. We’re here for you–whether you need help to manage your mental health, get a job, manage new medications, or find secure housing.

Depending on your needs, you may visit our program once per week or several times per month for appointments or meetings with your case manager. We will stay by your side as long as you need us.
